Many complex conditions are now being cared for in the home rather than in a hospital or skilled nursing facility. Our Home Care division brings comprehensive services delivered by a skilled and caring multidisciplinary team directly to you and your loved ones within the security and comfort of your home. Services range from providing assistance with bathing and dressing to performing complicated wound care and treatments, teaching about diabetes management, and providing therapy to regain strength, mobility and independence.
When is Home Care right for you?
Home Care may be appropriate if you or a loved one:
- Require assistance at home because of injury or surgery
- May be discharged from a hospital or nursing facility, but still need skilled care
- Need assistance to live independently or to regain strength and mobility due to disability, illness, or complications of aging
- Have conditions requiring skilled care and assessment such as congestive heart disease, diabetes, wounds or infections, and muscular or respiratory disorders
- Need home safety concerns addressed
- Experienced significant changes in your treatment plan and/or medications
- Have Intravenous Therapy or Enteral Nutritional (tube feeding) needs
What does Home Care consist of?
When Home Care services are requested, a physician’s order is required to initiate the process. A clinician is sent to your home to perform an initial assessment; based on this assessment, a treatment plan is developed to meet your individualized needs.
A variety of Home Care Team members such as nurses, medical social workers, home health aides and therapists may be involved in your plan of care. Our multidisciplinary team works closely with you and your physician in determining and providing the most appropriate services to help you recover and safely regain an optimum level of function.

Geographic Care Teams Offer Local Expertise
Our clinicians provide care within one of three multidisciplinary teams: the Seacoast area Team, the Derry/Salem area Team, or the Exeter area Team. Led by a Case Managing RN, teams are comprised of Skilled Nurses, Physical, Occupational and Speech Therapists, Medical Social Workers, and Home Health Aides. The Enterostomal Therapy Nurse, Registered Dietician and Certified Diabetes Educators work with members of each care team as needed.
This team approach is not only a more personalized way of providing care, it often leads to better results. We have found that patients and families are more comfortable with the knowledge that our clinicians work in their communities every day, bringing with them expertise about area resources, organizations and providers.
