
Patient & Caregivers - As the most important team members, patients and caregivers are instrumental in the development of the plan of care and in participating in the type of care they give/receive.
Home Care Medical Director oversees the Home Care Program and assists in the development of standards of care and evaluating clinical outcome data. The Home Care Medical Director of Rockingham VNA & Hospice, Margaret C. Tilton, MD, is Board Certified in Physical Medicine and Rehabilitation.
Skilled Nurses provide direct skilled care and assessment such as wound care, injections, care of the intravenous patient, and medication management. They also provide management of the plan of care, monitor the patient’s progress, and provide education to patients and caregivers.
Medical Social Workers assist patients and their families with the social and emotional factors affecting their condition, and help identify and access available community resources.
Home Health Aides help patients with activities of daily living such as bathing, dressing, toileting, and moving around. They also assist in following the nursing or rehabilitation plan of care.
Physical Therapists assess and work to restore the mobility and strength of patients who are limited or disabled by physical injury through the use of therapeutic exercises and other methods. They also educate patients and caregivers on maintaining safety while performing activities such as transferring and walking.
Occupational Therapists instruct individuals on the use of specialized rehabilitation techniques and adaptive equipment to improve their ability to safely perform activities of daily living such as eating, bathing, dressing, and basic household tasks.
Speech Language Pathologists work to develop and restore the speech of patients with communication disorders often resulting from stroke or injury. They also help to restore function by retraining in breathing, swallowing, and muscle control.
Registered Dieticians provide professional dietary assessment, guidance, and counseling services to help properly manage an illness or disability.
Clinical Specialists:
Enterostomal Therapy (ET) Nurse - A Registered Nurse certified in wound, ostomy, and continence care, the ET Nurse may be consulted to provide direct care, education and offer expertise on the treatment of individuals with wounds/pressure sores, surgical incisions, stomas, and incontinence problems.
The ET Nurse of Rockingham VNA & Hospice also helps to facilitate a weekly Ostomy Support Group in Exeter, NH.
Certified Diabetes Educators (CDE) - Registered Nurses certified in the management and treatment of diabetes, CDEs may be consulted to provide care, education, and offer expertise on the treatment of individuals with Type I or Type II Diabetes. The CDE is especially helpful in assessing and instructing patients and family members of newly-diagnosed diabetics, those with difficult to control blood glucose levels, and individuals who are adjusting or adding insulin to their diabetes management program.
The CDE of Rockingham VNA & Hospice also facilitates Diabetes Support Groups in our service area.
