
Rockingham Visiting Nurse Association & Hospice has developed Specialty Programs to address specific healthcare issues individuals and their families face.
Liaison Program: Smoothing the transition from inpatient to home care
On physician referral for Home Care, the Bridge Program or Hospice services, Rockingham VNA & Hospice team members meet with patients, family members, and health care providers at area hospitals and health care facilities, reviewing the care they will need, educating them about home health/hospice services, and facilitating their safe return home.
Patients and family members who have received liaison services have stated that they are better prepared for the transition to the home setting and are less anxious about what to expect from home health services.
Falls Risk-Reduction Program: Taking steps to reduce the risk of falls in the home
The Falls Risk-Reduction Program has been developed to assess and reduce the incidence of falls in elderly, at-risk populations. Patients are often referred for a Safety Evaluation Visit by physicians, first responders, or insurance companies. Referrals may also be made by family members, friends, or other health professionals. The Safety Evaluation Visit consists of:

A comprehensive evaluation performed by a skilled clinician

A Home Safety Assessment

Education on falls risk-reduction provided to patient and family members with appropriate environmental recommendations

Consultation and evaluation by other disciplines as needed
Complimentary educational programming is also available to community groups, providers, first responders, and other health facility personnel on falls risk-screening and reduction.
Orthopaedic PREOP Program: Preparing for joint replacement surgery
The Pre-operative Rehabilitation Services Program has been specifically designed for patients who are scheduled for elective hip or knee replacement surgery. Referrals are generally made from physician offices once the patient has been scheduled for their joint replacement surgery, and they involve:

A comprehensive Pre-operative Evaluation performed by a Physical Therapist

A Home Safety Assessment

Teaching and recommendations for assistive devices and adaptive equipment as necessary

A discussion of the surgical procedure & a review of possible post-operative tracks (Home Care, Outpatient, Subacute Care)

Instruction in an appropriate therapeutic exercise regime

Gait & transfer training
-
Physical Therapist is available to the patient & family members to act as a resource person throughout the pre & postoperative period

Disease Management Programs: Managing care to improve quality of life
Studies show that Disease Management provides an added capacity to care for individuals with certain diagnoses. Carefully planned interventions, patient and family education, and outcomes measuring are all components of this process. The result is often improved outcomes including a greater ability for an individual to manage their condition, an increased compliance to treatment, and a better quality of life.
